We welcome you as a patient and appreciate the
opportunity to provide you with medical services. The information
below is designed to introduce you to our services and to answer
your questions about our clinic and our policies.
Our physicians maintain active privileges at both Mercy
Medical Center and St.
Luke's Regional Medical Center in Sioux City, Iowa. Several of our
physicians provide care at The Siouxland Surgery Center as well as satellite
clinics throughout the Siouxland area.
Our offices are open Monday through Friday
from 8:00 am to 5:00 pm.
Patients are seen according to their appointment
time. All attempts are made to minimize waiting. However, emergencies
do occur and disrupt even the best schedule. Your patience is
appreciated during these times.
For your first appointment you will be asked to fill out information
forms. When possible, these forms will be sent to you by mail before your
appointment. Please fill these out completely, sign and date them. You
will be asked for a copy of your insurance card, so be sure to bring this
along. If you need assistance, please ask our receptionists. If your insurance
requires a referral by your primary care physician, this must be done
prior to your appointment with us. It is helpful if you bring any test
results, x-rays and a list of current medications to your appointment.
On all subsequent visits, please notify us of any address, phone number
or insurance changes.
The information in your chart is confidential
and the entire staff will protect that confidentiality. If you
wish to have your records released to an insurance company, lawyer,
another physician or any other person, you will need to sign an
authorization to release medical information. We will then send
the information for you.
You are expected to pay for your office
visit at the time of service. Payments can be made in the form
of cash, check, Visa, Master Card and Discover. We are always
willing to discuss fees with you and provide an estimate of charges
for your visit.
Statements are sent out monthly. If you have questions or are not able
to pay your bill within ten days of receipt of the statement, please call
our business office at 605-217-5500 to make arrangements. We do file insurance
claims for you and want to help you receive reimbursement from your insurance
company. However, please note, payment of the bill is the responsibility
of the patient and not a third party such as an insurance company or person
being sued. We cannot wait for settlement of lawsuits before being paid
for services. Paying your bill, in no way prevents or delays you from
collecting from your insurance company or third party payor.
